To make things easy for you, Delta Dental of Kentucky offers groups the option of paying their monthly premiums by an automatic withdrawal from a designated checking account.

What are the advantages?
With automatic withdrawals, your staff will not have to produce checks or rely on mail delivery. Your payment will be posted immediately to our system.

How does it work?
Delta Dental will withdraw the amount due from your checking account on the first business day of the month. For example, the bill you receive mid-May for your June premium would be withdrawn on June 1, the due date. 

Enroll Today

To enroll, simply download and complete the Authorization for Payment Form and fax to Delta Dental at (877) 439-8841, or mail with a voided/cancelled check from the bank account from which you want to have the payment withdrawn. Please make a copy of the form and keep with your contract.

Authorization forms received by the 10th of the month will activate withdrawal on the first of the next month.
